DILI is an adverse reaction to medications andother xenobiotics that leads to liver dysfunction. Based upon differentialhistological injury patterns, DILI is classified into hepatocellular,cholestatic and mixed ones; whereas hepatocellular DILI is associated withinflammation, necrosis and apoptosis, cholestatic DILI is associated with bileplugs and bile duct paucity. UDCA has been empirically used as a supportivedrug mainly in cholestatic DILI, but both curative and prophylactic beneficialeffects have been observed not only for cholestatic but also for hepatocellularDILI, according to preliminary clinical studies. This could reflect the factthat UDCA has a plethora of beneficial effects potentially useful to treat thewide range of injuries with different etiologies and pathomechanims occurringin both types of DILI, including anticholestatic, antioxidant,antiinflammatory, antiapoptotic, antinecrotic, mitoprotective, ER-stressalleviating, and immunomodulatory properties. In this review, a revision of theliterature has been carried out to evaluate the efficacy of UDCA in the wholeDILI spectrum, and these findings were correlated with the multiple mechanismsof UDCA hepatoprotection. This should help to better rationalize andsystematize the use of this versatile and safe hepatoprotector in each type ofDILI scenarios.
